Skúsenosti s betahistínom v liečbe závratových stavov (retrospektívne sledovanie). Nové trendy v liečbe vestibulárneho vertiga

Súhrn

Betahistín ako analóg histamínu so špecifickým mechanizmom účinku sa uplatňuje v liečbe vestibulárneho vertiga, tinitu a poruchy sluchu. Na základe dvoch retrospektívnych klinických ` sledovaní sa v práci hodnotil a) účinok betahistínu na zníženie frekvencie, intenzity a trvania vertiga, na tinitus a na poruchu sluchu u chorých s Ménie rovou chorobou; b) účinok betahistínu na obnovu periférnej vestibulárnej funkcie a dosiahnutie centrálnej vestibulárnej kompenzácie u chorých s vestibulárnou neuronitídou. Posudzoval sa efekt betahistínu v závislosti od výšky terapeutickej dávky a od dĺžky jeho podávania. Výsledky potvrdili vyššiu účinnosť terapeutickej dávky 48 mg/deň v porovnaní s dávkou 24 mg/deň pri všetkých sledovaných parametroch pri oboch diagnózach. Pri Ménie rovej chorobe sa aj po 6 mesiacoch liečby zaznamenalo zlepšenie vertiga a tinitu. Tolerancia plnej teraupeutickej dávky 48 mg/deň ` bola dobrá a pozorovali sa len ojedinelé vedľajšie účinky prechodného charakteru. Autori poukazujú na potrebu včasnej liečby vertiga s dostatočnou, t. j. plnou terapeutickou dávkou 48 mg/deň a odporúčajú jej podávanie po dobu 3 až 6 mesiacov v závislosti od charakteru choroby.

Summary

Betahistine as an analogue of histamine with a specific mechanism of action is used in the treatment of vestibular vertigo, tinitus and hearing impairment. Based on two retrospective clinical studies, the following indicators were evaluated: a) the effect of betahistine on the reduction of frequency, intensity and duration of vertigo, on tinitus and on hearing impairment in patients with Ménier's disease, b) the effect of betahistine on the renewal of peripheral vestibular function and the accomplishment of central vestibular compensation in patients with vestibular neuronitis. The effect of betahistine depending on the level of therapeutic dosage and the time of administration was evaluated. The results confirmed higher efficiency of the therapeutic dose of 48 mg/day as compared with the 24 mg/day dose with all the monitored parameters in both the diagnoses. In the case of Ménier's disease, improvement of vertigo was also recorded after 6 months of treatment. The tolerance to full therapeutic dose of 48 mg/day was good and few adverse events of transitory nature were observed. The authors point out the need of timely treatment of vertigo with sufficient, i.e. full therapeutic dose of 48 mg/day, and recommend its administration for a period of 3 to 6 months depending on the nature of the disease.
